Day 5: Today's goals March 18, 2012

Happy Sunday morning to you all!

We thank all of you who are praying and fasting today and giving an even greater spiritual effort on Scott's behalf today.  Ashley and I are planning our own sacrament service, as well, to help boost our emotional and spiritual strength.  We know that the additional collective efforts will be helpful in so many ways.  All of these efforts in Scott's behalf also help us tremendously.  We have gained so much joy and strength from reading comments, emails, facebook posts, etc.  And... as we share the comments with each other, we share special stories about each of you, remembering how you are so special to Scott and why he loves you so much.  I wish that we could share those thoughts with each one of you.  Even when we have been able to return a short message, it is just that..short... Time is at a premium.  But... hopefully we will get more of an opportunity to share with each of you the ways in which we know Scottie loves you, cares for you, and enjoys you.  He is a man with a large heart with a great capacity to love his fellow being, so his feelings for his friends and family are strong and deep.  It gives us great comfort and joy seeing all of the people's lives that Scottie has touched.  So... thank you for your comments, prayers, and well wishes.  They are bringing us some joy during this difficult time.

Medically... these are our goals for today:
1.  When 24 hours after his surgery yesterday have passed, they will begin:

       a)  "vent trials" - they will try weaning him off of artificial ventilation and allow him work toward breathing on his own.  The machine will still help to maintain an appropriate air pressure to fully fill the lungs, but he will breathe on his own.  Initially, he was on 35% oxygen.  The air you and I breathe is 21%.  He is at 25% today.  They have also taken him off of supported ventilation.  He is breathing on his own.  If he does not take a breath in 20 seconds, the machine will kick on, just in case.  (Sorry, I can't get this to tab over to make it more readable.)

       b)They took chest x-rays again this am, but we have not heard the results from the MD yet.   Once he can go 24 hours without a pneumothorax, they will remove the chest tube.  That would be cool to take a tube away rather than adding tubes, as they have been.

       c) They will also begin reducing his sedation and work toward waking him up.  This should definitely be in everyone's prayers!

       d)  Get his bowels moving.  He has not had a bowel movement yet, which is also a common side effect of his body being "opiate naiive", as they call it.  He has not responded to previous attempts, so they will be pulling out the "Big Guns" today to assure it happens.

      e) They will also transition his central line to a PIC line, which is a better, less infection risk, long-term option.


Current Victories for the day:
1.    His blood sugar has stabilized.  Elevated blood sugar is a common stress response, so they have had to deliver insulin to him to establish some homeostasis.
2.  He is breathing on his own and the oxygen concentration has been reduced to 25%.
3.  The respiratory therapist says that the x-rays look better than yesterday.  Still haven't heard from them MD.
4.  The sedation (Propofol) is reduced to 20 mcg/kg/min currently.  (We may have to tie his left wrist down in order to progress the reduction plan.  He becomes much more active with the sedation decreased, and he likes to pull on all of his tubes.)
5.  His temperature remains normal.  No more 109 degrees! Yeah!
6.  They will be more aggressive with him today.  Physical Therapy will even start today.


Current Challenges:
1.  Still not awake.... first "wake assessment" of the day = no convincing change.
2.  BP has remained high (160s/low 70s) since this last surgery, but they are not worried about it, saying it is normal following general anesthesia.
3.  No bowel movement yet.

Okay,... Ashley is making me laugh.  She just walked back in the room after freshening up.  Her hair is done pretty, her makeup is on, ...she looks mega cute, as you might imagine.  Her response to me telling her how beautiful she looks,.... "Well, I gotta' look good for him if he is going to wake up today!  Especially if he doesn't remember me... (which is a cruel but distinct possibility).... I need to make him fall in love with me all over again."  You are missing her cute tone of voice with this statement, but it was playful and fun, not sad and morose.

Okay,... that is all I can remember for the morning update....  I expect today to be a busy day.  Lotsa' goals to accomplish in one day..... I also know that we will see the benefit from all of the extra prayers and fasting today.

Oh... I almost forgot.... Ashley's sister, Bree, and Scott's sister, Andrea, have set up Recovery accounts in Scott's name at America First (Utah) and Wells Fargo (everywhere else).  So many of you have pleaded to be able to do something tangible to help.... wanting to send food and flowers, wanting to help with logisitics here at the hospital and away at our neglected homes, wanting to fly in to help, etc....  We would request in lieu of these other tangible offers of help, that those who are feeling a "need" to help in some way, consider contributing to this fund.  Prayers are what we need the most right now!  And we are so eternally grateful for all that we have received!  Seriously, we have been so overjoyed with the amount of love shown to Scott and our family.

This journey will be a long one, though.  His recovery will take a long time...there will be many additional expenses... and we have been told that Scott will likely lose his only source of income, since his military status is now suspended and he was student/not considered active duty/no base to report to....  As everyone knows, Scott and Ashley have prepared well for a rainy day, but this situation could still be overwhelming and devastating to them.
